By participating in this plan, you acknowledge that your account may be transferred, serviced, or collected by an authorized third party. This does not alter your obligations or the total amount due. Your responsibility to make timely payments remains the same.

By signing below, I understand and agree to the following:

1. The income information I provide will be used solely to determine eligibility for a tuition payment plan administered by Dolly Monroe Beauty Academy or its third-party financing partners.

2. My information may be shared with third-party financing companies or factoring firms for the purpose of verifying income, servicing my account, or collecting payment.

3. I authorize Dolly Monroe Beauty Academy and its authorized partners to contact me regarding my application using the information I’ve provided.

4. I certify that all documents and information submitted are accurate to the best of my knowledge.

5. I have received and signed the Income Verification & Consent Form.

6. I agree to the terms listed above and understand my financial responsibilities.

7. For dual enrollment students you are applying for two separate financed accounts and therefore two separate contracts will be generated along with any fees.

8. First Payment Due Date & Contract Activation:The first payment under this financing agreement is due thirty (30) days after the date of execution of the enrollment contract. If the class start date selected by the student occurs within this initial thirty (30) day period, the student’s enrollment will be deferred to the next available class start date following the payment due date. The student may, at their sole discretion, remit the first payment prior to the thirty (30) day due date in order to begin with an earlier start date. The institution shall not require or encourage early payment; however, the enrollment contract shall not be considered fully active, and a seat shall not be reserved, until the first payment has been received in full.
